MSP ENROLLMENT: WHO MUST ENROLL?

CU employees, students and volunteers with exposure to animals, animal tissue or waste, or human blooborne pathogens (human blood or other potentially infectious materials) must enroll in the Medical Surveillance Program (MSP). Principal Investigators (PI's), Advisors and Supervisors of individuals with these exposures must require their personnel to enroll in the Medical Surveillance Program and to complete and document the appropriate training (Zoonotic, OSHA Bloodborne Pathogens Standard, "Hantavirus," as needed.) 

If requested, a University Veterinarian or the Occupational Health Nurse will provide additional zoonotic information or training material to instructors of animal science courses and farm managers for their classes. 

ADDITIONAL TRAINING REQUIREMENTS FOR SOME EXPOSURES: 

  • EXPOSURE TO HUMAN BLOOD, TISSUE, CELLS, OR OTHER POTENTIALLY INFECTIOUS MATERIALS: OSHA Bloodborne Pathogens Standard training is required annually for persons with human biohazard exposure.  Training can be taken online at the EHS website, with documentation accomplished online after completion of the module.  By request, the Clemson occupational health nurse is available to give group presentations to departments or facilities needing OSHA Bloodborne Pathogens Standard training. Persons who have already had the Hepatitis B series (3 immunizations) or who do not wish to receive these at this time may submit an OSHA Hepatitis B Decline form.
  • TRAPPING OF SMALL WILD ANIMALS: persons planning to trap small wild animals will need to contact the OHN to arrange for "hantvirus" training. This consists of specific information, medical clearance for respirator use, respirator fitting through EHS, and special training by Research Services personnel. All participants in the protocol should plan to attend this session together, and it can usually be completed in a few  hours with advance planning.  If you will be trapping small wild animals, please print out and study the Clemson University Policy for Individuals Participating in the Trapping or Handling of Small Animals in Field Studies and bring your copy with you for your appointment with the Occupational Health Nurse. Please also submit a completed OSHA Respirator Questionnaire with your other enrollment forms. 

THE MSP "ENROLLMENT" PROCEDURE: 

  • If an immunization or additional training is needed, please call 656-3076 for an appointment for "MSP enrollment," but please send your forms in advance if possible.
  • If no immunization or additional training is required, enrollment in the Medical Surveillance Program may be accomplished by printing and completing the 3 forms below and then submitting them by FAX, "campus" or U.S. Mail, or by bringing them to the Medical Surveillance Program office:

  • Enrollment and training forms will be reviewed by the occupational health nurse. Please be sure to include your email address and office or home phone number on the "Client Profile" form so you can be easily reached.
  • You will be notified that your enrollment is complete or that it is "pending" other services.
  • If you are participating in an ARC "Animal Use Protocol" (AUP) or an IBC "Biohazard Protocol," a Medical Surveillance Program certificate of completion will be sent to the Office of Research Compliance as soon as enrollment and training requirements for the protocol have been met.
  • You will be considered "enrolled" in the MSP for your current exposures or protocol after the above process has been accomplished.  If you change animal species during your affiliation with Clemson, you will need to complete zoonotic training appropriate for your new species and submit documentation of the new species training.
  • Individuals who are exposed to unique hazards or hazardous agents may be required to receive further training, testing, or surveillance as determined by current occupational and environmental medical recommendations. 

 

FORMS TO PRINT, COMPLETE, AND SUBMIT FOR MEDICAL SURVEILLANCE PROGRAM ENROLLMENT:

 

1. Client Profile demographic information.  If your name changes during your affiliation with the University (i.e., due to marriage, etc.), please notify the MSP receptionist at (864) 656-3076 so we can keep your records current. 

2. Occupational Health History -- will be evaluated to determine risk associated with your animal, chemical, biological, or physical exposures.  Be sure to complete all sections, with particular attention to the following:

  • Date of last Tetanus (Td) booster  You must submit the date (or an approximation) of your last Tetanus/Td booster as part of the MSP enrollment process.  Redfern Health er may be able to provide your Td date if you submitted an immunization record to them.  CU employees who work with animals/animal tissue, human biohazards, or soil and know or suspect that it has been >10 years since their last Td booster can receive a free Td booster through the Medical Surveillance Program.  Please call 656-3076 for an appointment. 
  • History of allergies, including food, medication, animals, dermatitis, nasal allergy, asthma, latex sensitivity, insect sting allergy, etc.

3. Zoonotic Training Documentation Form -- complete this form after you have read and understood the zoonotic information pertaining to each species with which you will be working.  Submission of this form will indicate your affirmation that the information has been read and understood.

Women Planning a Pregnancy should consult with their physician regarding exposures that may present a unique hazard in pregnancy.
